乳腺癌腋窝淋巴结隐匿转移与肿瘤预后的关系

目的：探讨免疫组化在识别单个或微小癌巢的价值，及乳腺癌淋巴结匿转移与预后的关系。方法，选用ＨＥ切片光镜检查无腋窝淋巴结的乳腺浸润性导管癌８８例，对其腋窝淋巴结共８２３个进行光镜复查及免疫组化检查，抗体为角蛋白Ｃｙｔｏｋｅｒａｔｉｎ，并将检查结果与肿瘤大小、肿瘤内淋巴管癌栓、术后５年存活率进行对照分析。结果８８例中１９例（２１．５９％）共４１个（４．９８％）淋巴结发现Ｃｙｔｏｋｅｒａｔｉｎ阳性的隐匿

The Relationship Between Prognosis and Occult Metastasis of Axillary Lymph Node in Breast Cancer

Objective:To study the relationship between the prognosis and occult metastasis of axillary lymph node.Methods:88 cases of invasive ductal carcinoma without axillary lymph node metastasis were selected,and 823 axillary lymph nodes were immunohistochemically studied.Results:Among the total 823 axillary lymph nodes of the 88 cases,41 nodes of 19 cases were found to have cytokeratin positive occult metastatic cancer cells.Among the 19 cases intratumoral lymphatic emboli were found in 8 cases,while among the other 69 occult metastasis negative cases,only 3 cases had lymphatic emboli( P <0.01).7 of the 19 cases with occult metastasis died within 5 years after operation;while only 3 of the other 69 cases without occult metastasis died within 5 years after operation( P <0.01).Conclosion:The immunohistochemical method is helpful to find the axillary lymph node metastasis in breast cancer and therefore,is useful for clinical treatment.
