恶性Wolffian附件肿瘤的临床病理分析

目的:探讨午非附件肿瘤的临床病理和生物学行为。方法:对1例恶性午非附件肿瘤进行临床病理分析并复习文献。结果:恶性午非附件肿瘤位于右侧输卵管系膜,肿瘤包膜多结节,与网膜腹壁粘连,肿瘤组织弥漫实体、管状、筛状结构,内衬多角形和短梭形细胞,细胞异型核深染,核分裂多见,出血坏死明显。免疫表型AE1/AE3、Vim阳性,Ki6740%阳性。结论:大多数午非附件肿瘤为良性病变,然而较为罕见的情况下可见恶性病例,镜下明显细胞多形性及多见核分裂、部分侵袭性现象如复发和转移,提示其恶性可能。

Clinicopathologic analysis on malignant Wolffian adnexal tumor

Objective：To investigate the clinicopathologic features of Wolffian adnexal tumor（WAT）and its biological behavior.Methods：One malignant case of WAT was examined retrospectively in terms of clinicopathologic findings and involved literatures.Results：The tumor arose from the right lateral mesosalpinx and was characterized by multiple nodes in tumor capsule,adhesion to omentum and peritoneum,diffused tumor cells and solid sheets,with packed tubules or cribriform structure and lined by polygonal and spindle-shaped cells,cellular atypia with hyperchromic nucleus,intensive mitotic figure and visible hemorrhage and necrosis.Immunohistochemistry showed positive AE1/AE3,Vim and Ki67（40%）.Conclusion：WAT is benign lesion in most cases,and yet,malignancy may be rare.Microscope commonly exposes cellular pleomorphis with karyokinesis.Given partial invasion such as relapse and metastasis,the condition seems potential sign of malignancy.
