经阴道彩色多普勒超声在诊断早期异位妊娠的临床价值

目的:探讨经阴道彩色多普勒超声在诊断早期异位妊娠的临床价值。方法:选取2019年7月~2020年7月沈阳经济技术开发区人民医院妇产科就诊的早期异位妊娠患者68例,对所有患者均行经阴道彩色多普勒超声检查与经腹部彩色多普勒超声检查,以术后病理学结果为金标准,比较经阴道彩色多普勒超声检查与经腹部彩色多普勒超声诊断早期异位妊娠的效能。结果:经阴道彩色多普勒超声诊断早期妊娠的准确率为91.2%(62/68),明显高于经腹部彩色多普勒超声诊断早期妊娠的准确率75.0%(51/68),两种诊断方法差异相比有统计学意义(P<0.05)。经阴道彩色多普勒超声在胚芽、宫腔内假胚囊、附件区团块、原始心管搏动、盆腔积液显示率高于经腹部彩色多普勒超声,两种诊断方法相比差异具有统计学意义(P<0.05)。结论:经阴道彩色多普勒超声分辨率高,能很好地观察孕囊情况,诊断早期异位妊娠具有较高的准确率,能为临床医生准确地提供诊断信息,能为后续的治疗提供精准的科学依据,具有重要的临床参考价值。

Clinical Value of Transvaginal Color Doppler Ultrasound in the Diagnosis of Early Ectopic Pregnancy

Objective:To explore the clinical value of transvaginal color Doppler ultrasound in the diagnosis of early ectopic pregnancy.Methods:68 patients with early ectopic pregnancy in the Department of Obstetrics and Gynecology of the People’s Hospital of Shenyang Economic and Technological Development Zone from July 2019 to July 2020 were selected.All patients were examined by transvaginal color Doppler ultrasound and transabdominal color Doppler ultrasound.The postoperative pathological results were taken as the gold standard,to compare the efficacy of transvaginal color Doppler ultrasonography and transabdominal color Doppler ultrasonography in the diagnosis of early ectopic pregnancy.Results:The accuracy of transvaginal color Doppler ultrasound in the diagnosis of early pregnancy was 91.2%(62/68),which was significantly higher than the accuracy rate of 75.0%(51/68)in the diagnosis of early pregnancy by transabdominal color Doppler ultrasonography.There was significant difference between the two diagnostic methods(P<0.05).The display rates of embryo,intrauterine pseudoembryo sac,adnexal mass,primitive cardiac pulsation and pelvic effusion in transvaginal color Doppler ultrasound group were higher than those in transabdominal color Doppler ultrasound group.The difference between the two diagnostic methods was statistically significant(P<0.05).Conclusions:Transvaginal color Doppler ultrasound has high resolution,can well observe the gestational sac,has high accuracy in the diagnosis of early ectopic pregnancy,can provide accurate diagnostic information for clinicians,and can provide accurate basis for hemoptysis for follow-up treatment.It has important clinical reference value.
