Efficacy of Duhuo Jisheng decoctioncombined with thunder-fire moxibustion in the treatment of knee osteoarthritis of wind-cold-dampness arthralgia type and its effect on joint function and serum MMP-3 and osteopontin levels

Objective To explore the efficacy of Duhuo Jisheng decoction combined with thunder-fire moxibustion on knee osteoarthritis(KOA)of wind-cold-dampness arthralgia type and its effect on joint function and serum matrix metalloproteinase-3(MMP-3)and osteopontin(OPN)levels.Methods A total of 120 KOA patients admitted to Haikou Hospital of Traditional Chinese Medicine from January 2020 to June 2021 were prospectively selected and randomly divided into the control group and the observation group according to the random number table method,with 60 cases in each group.The control group was treated with celecoxib combined with glucosamine sulfate,and the observation group was treated with Duhuo Jisheng decoction combined with thunder-fire moxibustion.The clinical efficacy,and TCM syndrome score,range of motion(ROM),swelling degree,joint functionwestern ontario and mcMaster universities osteoarthritis index(WOMAC)],serum interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),MMP-3 and OPN levels before and after treatment were compared between the two groups,and the adverse reactions were observed.Results The total effective rate of the observation group was significantly higher than that of the control group(91.67%vs.78.33%),and the differences was statistically significant(P<0.05).Compared with the control group,the swelling degree of knee joint,TCM syndrome score,pain,stiffness and daily activity score in WOMAC in the observation group were significantly lower,and ROM was higher,the differences were statistically significant(P<0.05).The levels of serum IL-6,TNF-α,MMP-3 and OPN in the observation group after treatment were(122.35±26.75)pg/mL,(2.13±0.66)mg/L,(1.25±0.36)mg/L,(26.87±7.34)mg/L,which were significantly lower than those in the control group(164.34±33.21)pg/mL,(3.55±0.72)mg/L,(1.47±0.38)mg/L,(33.58±7.89)mg/L],the differences were statistically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(5.00%vs.8.33%,P>0.05).Conclusion Duhuo Jisheng decoction combined with thunder-fire moxibustion has good efficacy and safety in the treatment of KOA with wind-cold-dampness arthralgia syndrome,which can effectively relieve clinical symptoms,improve joint function,reduce inflammatory response,and reduce serum MMP-3 and OPN levels.
