促凝血活性试验诊断风湿性心脏炎意义的再评价

目的：为了寻找特异性诊断风湿性心脏炎的方法。方法：测定５５例风湿性心脏炎患的外周血淋巴细胞促凝血活性（Ｐｅｒｉｐｈｅｒａｌ ｂｌｏｏｄ ｌｙｍｐｈｏｃｙｔｅ ｐｒｏｃｏａｇｕｌａｎｔ ａｃｔｉｖｉｔｙ，ＬＰＣＡ），并了解他们的外周血 巴细胞对体外培养的人胚心肌细胞促凝血活性（Ｈｅａｒｔ ｔｉｓｓｕｅ ｃｅｌｌ ｐｒｏｃｏａｇｕｌａｎｔ ，ＨＰ－ＣＡ）的影响。ＨＰＣＡ的测定以血浆凝固时间为指标。结果：风湿性心脏炎患的外周血淋巴细胞能使ＨＰＣＡ明显升高，与其他对照组有显性差异（Ｐ〈０．００１），而宜综的改变与ＬＰＣＡ有高度的一致性，ＨＰＣＡ及ＬＰＣＡ在风湿性心脏炎时的慢性率分别为８０．０％、８３．６％，均高于ＥＳＲ和ＣＲＰ。结论：揭示ＨＰＣＡ和ＬＰＣＡ均能较特异和敏感地反应湿性心炎，其中ＬＰＣＡ可作为风湿性心

Further study of procoagulant activity assay in the diagnosis of rheumatic carditis

Objective To further study the significance of procoagulant activity in diagnosis of rheumatic carditis (RC). Methods Lymphocyte procoagulant activity (LPCA) was tested in 55 cases with RC. After stimulated by peripheral blood lymphocyte of RC, procoagulant activity of embryo human heart tissue cells in culture was also tested. Heart tissue cell procoagulant activity (HPCA) was measured by using one stage plasma recalcification clotting time. Results Peripheral blood lymphocytes of cases with RC can make increase in HPCA in vitro compared with those of controls ( P ＜ 0.001 ). HPCA has high positive correlation with LPCA in RC. The positive rates of HPCA an LPCA were 80.0 ％ and 83.6％ in RC respectively, both higher than that of ESR、 CRP. Conclusion Both LPCA and HPCA can be a indicate for the diagnosis of RC, and LPCA has good specificity, it is valuable to extand usage.
