左向右分流型先天性心脏病患儿潮气呼吸肺功能的改变

摘    要:【目的】探讨左向右分流型先天性心脏病(简称先心病)患儿肺功能的改变。【方法】收集于2006年1月至2008年12月在本院住院行手术治疗的52例左向右分流型先心病患儿为先心病组,同时设置健康对照组50例。健康对照组与先心病组分别进行潮气呼吸肺功能测定。【结果】先心病组患儿潮气呼吸肺功能与健康对照组相比,每分通气量(MV)差异无显著性(P〉0.05),呼吸频率(RR)增快,潮气量(VT/Kg)减小(P〈O.05);吸气时间(TI)及呼吸比(TI/TE)缩短(P〈0.05);达峰时间(TPEF)、达峰时间比(TPEF/TE)、达峰容积(VPEF)及达峰容积比(VPEF/VE)均明显降低(P〈0.05);潮气呼气峰流速(PTEF)增快(P〈0.05),潮气量时呼气流速(TEF25%)减低(P〈0.05),而TEF75%、TEF50%差异无显著性(P〉0.05)。【结论】左向右分流型先心病可影响肺功能,引起限制性及阻塞性通气障碍。

Abstract:To explore the changes in pulmonary function of pediatric patients with left-to-right shunt congenital heart disease (CHD). [Methods] From January 2006 to December 2008, the 52 children with left-to-right shunt congenital heart disease (CHD) who underwent operation in this hospital were selected to be CHD group and 50 healthy children were considered to be control group. Pulmonary function of tidal respira- tion was tested in both groups. [Results] CHD children had worse pulmonary function of tidal respiration compared with healthy ones, showing that RR and PTEF increased ( P 〈0.05) and VT/Kg, TI, TI/TE, TPEF, TPEF/TE, VPEF, VPEF/VE and TEF25% decreased ( P 〈0.05). However, MV, TEF75% and TEF50% in C HD group were not significantly different from those in control group ( P 〉 0.05). [Conclu sion] Left-to-right shunt congenital heart disease can influence children's pulmonary function of tidal respiration and result in restrictive and obstructive pulmonary ventilation dysfunction.
