改良真皮帽双环法行乳房缩小整形术

目的：灵活应用真皮帽双环法行乳房缩小成形。方法：采用传统的双环法设计切口，外下象限辅助S形切口线，保留乳晕周围较大面积完整的真皮帽，切除乳房外下象限腺体组织，设计外上及内下两个腺体组织瓣交错连接固定成形，乳晕周围内外环真皮荷包缝合，修整乳晕周围及外象限多余皮肤，切口呈环形，辅助切口瘢痕较小。结果：术后三月随访，新乳房的大小和形态．乳头乳晕的位置和形态均满意，除具有良好的感觉及勃起功能外，还保留了泌乳功能。结论：用组织瓣旋转固定的改良真皮帽双环法行乳房缩小成形，手术操作简单，组织瓣及乳头乳晕供血丰富，组织损伤小，值得推广。

Modified reduction mammaplasty of applying the dermal cap and bi- loop method

Abjective: To Apply the method of dermal cap and bi -loop to reduction mammaplasty. Methods: Applying the conventional bi-loop method to design incisions, adding to an accessorial S-form incision at lateral inferior part of breast. Withhold whole piece and biggish size of dermal cap surrounding areola of breast and resect mammary gland at lateral inferior part of breast. Design two pieces of mammary gland tissue flap from lateral superior part and medial inferior part of breast, then connect and fix them to take a good shape by staggering the two pieces of flap. Make the derma between the inside loop and outside loop of surrounding areola of breast be purse -string sutured. Eliminate unwanted skin of surrounding areola of breast and lateral quadrant of breast. Incisions take on a loop form. The scar of accessorial incision is lesser. Results: In the follow-up after postoperative three months, the size and shape of new breasts and the location and shape of nipple and areola of breasts were satisfying, the breasts not only had satisfactory sensory function and erection function but also reserved lactation function. Conclusion: By means of improved reduction mammaplasty of applying the method of dermal cap and bi -loop and tissue flap rotation and fixation, operational procedure is simple, the blood supply of tissue flap and nipple and areola of breast is plentiful and impairment of tissue is light, so it is good for generalization.
