联合应用宫腹腔镜行输卵管插管和应用透明质酸钠治疗输卵管性不孕的疗效观察

目的 观察联合应用宫腹腔镜行输卵管插管和应用透明质酸钠治疗输卵管性不孕的临床效果.方法 将120例经输卵管造影或腹腔镜确诊为输卵管性不孕症的患者,并随机分为3组,每组各40例,A组行腹腔镜下输卵管整形术、宫腔镜下输卵管插管术联合应用透明质酸钠治疗;B组行腹腔镜下整形术、宫腔镜下输卵管插管术治疗;C组输卵管造影及输卵管阻塞介入治疗.结果 3组患者宫内妊娠、异位妊娠、未妊娠的情况相比较差异无统计学意义（P＞0.05）;A组的术后输卵管通畅、受孕率明显高于B、C组（P＜0.05）,A组的术后输卵管阻塞明显低于B、C组（P＜0.05）,而B组的输卵管通畅及受孕率情况与C组比较均差异无统计学意义（P＞0.05）.结论 联合应用宫腹腔镜行输卵管插管和应用透明质酸钠治疗输卵管性不孕患者有协助作用,具有准确率高、手术成功率高、创伤小、恢复好、术后受孕率高的特点,值得临床推广.

Curative effect observation on laparoscopy combined with hysteroscopy tubal intubation and the application of sodium hyaluronate in the treatment of fallopian tube infertility

Objective To observe the effect of combined application of palace laparoscopic tubal intubation and the clinical effect of application of sodium hyaluronate in the treatment of tubal infertility.Methods 120 cases of fallopian tube diagnosed by angiography or laparoscopy for tubal infertility were randomly divided into 3 groups,40 cases in each group,group A underwent laparoscopic tubal surgery,hysteroscopic tubal cannulation of the combined application of sodium hyaluronate in the treatment; group B underwent laparoscopic surgery,hysteroscopy tubal intubation treatment; group C salpingography and fallopian tube obstruction interventional therapy.Results There was no statistical significant difference found in 3 groups of patients with intrauterine pregnancy,ectopic pregnancy and non-pregnancy （P ＞ 0.05）;postoperation smooth and the pregnancy rate in group A were significantly higher than those in group B and C （P ＜ 0.05）,group A＇ s fallopian tube obstruction was significantly lower than those in group B and C （all P ＜ 0.05）,and there was no significant difference in tubal patency and pregnancy rate in group B and C （P ＞0.05）.Conclusion Laparoscopy combined with hysteroscopy tubal intubation and the application of sodium hyaluronate in the treatment of patients with tubal infertility can complement each other,with characteristics of high accuracy,small trauma,good operation recovery,high success rate and high postoperative pregnancy rate,which is worth of the clinical promotion.
