革兰染色法与湿片法诊断细菌性阴道炎结果比较

目的通过两种方法检出率的比较,评价革兰染色法在阴道分泌物检验中的应用价值。方法对4 656例妇科门诊病人的阴道分泌物同时用革兰染色法和湿片法进行检验,按临床基础检验学的诊断标准,观察各种病原体的形态进行诊断,并对两种方法检测结果进行比较。结果革兰染色法假丝酵母菌阳性率为35.8%,线索细胞阳性率为26.4%,纤毛菌阳性率为8.0%,毛滴虫阳性率为6.5%,G-双球菌阳性率为1.5%;湿片法假丝酵母菌阳性率为26.2%,线索细胞阳性率为20.5%,纤毛菌阳性率为4.5%,毛滴虫阳性率为4.2%,G-双球菌阳性率为0,两种方法比较差异有统计学意义（χ2=25.2-547.3,P〈0.01）。结论革兰染色法较湿片法可更全面检出引起妇科炎症的病原体及菌群的分布状态,为临床诊断提供可靠依据。

Results of detection of pathogen from 4 656 vaginal scretion samples by Gram-staining and wet mount methods

Objective To assess the value of Gram-staining and wet mount methods in detection of pathogens from vaginal secretion.Methods he 4656 vaginal secretion samples from outpatients were detected simuteneously by using Gram staining and wet mount methods.Morphological disgnosis of various pathogens was made based on the basical clinical test standard.Results The positive rates of Candida krusei,clue cell,Leptotrichia,Trichomomonas vaginalis and G-diplococcus were 35.8%,26.4%,8.0%,6.5% and 1.5% by Gram staining method.While the positive rates were 26.2%,20.5%,4.5%,4.2%,and 0 by wet mount method,showing significant differneces between the two methods（χ2=25.2-547.3,pall〈0.01）.Conclusion The Gram staining methods is suitable for detection of distribution of pathogens from vaginal secretions,thus it is recommended as the routine examination mehod for detection of vaginal secretion.
