SPECT测定GFR在2型糖尿病中的应用

目的通过SPECT测定GFR提高临床上糖尿病肾脏疾病患者检出率。方法门诊或住院的45例2型糖尿病患者。正常对照组15例。对患者进行中医辨证分型,并设立正常对照组。行SPECT测定GFR检查,24 h尿微量白蛋白测定,血肌酐及血尿素氮检查。结果①阴虚热盛,气阴两虚及阴阳两虚证三组患者Alb及GFR相比,均P〈0.05,差异有统计学意义。②〈30 mg/24 h组患者GFR与正常组对比升高,P〈0.05,差异有统计学意义。③〈30 mg/24 h组与30～300 mg/24 h组患者Scr、BUN均在正常范围;〉300 mg/24 h组该值明显升高,与前两组相比,P〈0.05,差异有统计学意义。结论SPECT测定GFR可早期发现糖尿病肾脏疾病患者。阴虚热盛、气阴两虚及阴阳两虚证反映了糖尿病中医病机演变的过程。

Application of SPECT determing GFR in type 2 diabetes mellitus

Objective Application of the detection of GFR by SPECT in type 2 diabetes mellitus objec tive GFR measured by SPECT improve the clinical detection rate of patients with diabetic nephropathy. Methods There were outpatients or Hospital outpatients of 45 patients with type 2 diabetes mellitus. The normal con trol group, there were 15 cases of healthy people or health volunteers. Patients were divided into three groups by syndrome differentiation, and the establishment of the normal control group. Patients with GFR measured by SPECT examination, a 24hour urinary albumin measurement, blood creatinine and blood urea nitrogen. Results （1）Alb and GFR of traditional Chinese medicine syndrome differentiation three groups compared with both P 〈 0. 05, statistically significant. （2）GFR in patients 〈 30 mg/24 h was increased compared with the normal group, P 〈 0. 05, there were significant differences. （3）The first two groups in patients with Scr, BUN were within the normal range, and Scr, BUN of the 〉 300 mg,/24 h group was significantly increased compared with the previous two groups, P 〈 0.05, there are statistically significant. Conclusion SPECT measured GFR can found the ear ly diabetic nephropathy. The three Syndrome of TCM reflect the evolution of Pathogenesis of TCM.
