腹腔镜可调节胃捆扎带术治疗单纯病态肥胖症的临床应用

目的:评价腹腔镜可调节胃捆扎带减容术(laparoscopic adjustable gastric banding,LAGB)治疗单纯病态肥胖症的临床效果及安全性。方法:我科2005年8月~2007年4月应用瑞典可调节胃捆扎带(Swedish adjust-able gastric band,SAGB)对4例病态肥胖患者施行LAGB,并观察其初步疗效。结果:4例手术均成功进行,其中1例联合行胆囊切除术,手术时间70~170min,出血量10~50ml,无手术死亡及术中并发症,手术当日麻醉苏醒后下床活动,术后住院2~7d。术后出现套管孔感染、饭后呕吐各1例,均经对症处理治愈。术后1例随访24个月,减重15kg。3例随访3个月,减重9~14kg。全组患者均未出现营养不良症状。结论:LAGB治疗单纯病态肥胖症具有微创、安全、有效、可调节、可恢复性等特点,近期减重效果满意,但其远期疗效仍需进一步观察。

Laparoscopic adjustable gastric banding in treatment of morbid obesity

Objective: To evaluate the effects and safety of laparoscopic adjustable gastric banding (LAGB) in treatment of morbid obese subjects. Methods: From August 2005 to April 2007,four patients underwent Swedish adjustable gastric band (SAGB) in our department. Results: All procedures were successfully performed and the combined procedure including cholecystectomy in one case. The range of the operative time was 90~170 minutes, the intraoperative blood loss were approximately 10 to 50 millilitres. Postoperative hospital stay were ranged from 2 to 7 days. There were no postoperative deaths and severe complications, and one case of access-port infection and one case of vomiting occured.The follow-up was 24 months for first patient and 3 months for other 3 patients, the weight loss were 15 kg and 9~14 kg, respectively. Conclusions: LAGB would be minimal invasive, safe, effective and adjustable procedure for morbid obesity, but its long term efficacy is to be investigated.
