紫杉醇脂质体与传统紫杉醇治疗乳腺癌的疗效分析

目的 比较注射用紫杉醇脂质体与传统紫杉醇注射液治疗晚期乳腺癌的疗效及不良反应。方法 采用随机对照的方法进行研究，晚期乳腺癌患者62例随机分为试验组和对照组。试验组（31例）给紫杉醇脂质体每次剂量135mg／m^2；对照组（31例）给紫杉醇每次剂量135mg／m^2。两组均联合阿霉素或表阿霉素治疗，每3周重复1次为1周期，共2周期。结果 入组62例患者中，有60例可供评价不良反应，60例可供评价疗效。试验组30例，完全缓解率10％，部分缓解率36．7％，总有效率46．7％；对照组30例，完全缓解率3．3％，部分缓解率40．0％，总有效率43．3％。两组疗效差异无统计学意义。在血液学毒性方面，两组发生率差异无统计学意义。在聚氧乙基代蓖麻油与无水乙醇的混合溶媒所产生的相关毒性方面，试验组发生率明显低于对照组，差异有统计学意义。结论 紫杉醇脂质体联合阿霉素或表阿霉素治疗晚期乳腺癌效果良好，两种紫杉醇的疗效相当，但其溶媒所产生的皮疹等过敏反应，紫杉醇脂质体明显低于紫杉醇注射液，其余不良反应两组相仿。

Effects of paclitaxel iiposomal and traditional paclitaxel in the treatment of advanced breast cancer

Objective To analyze the effects and side effects of paclitaxel liposomal formula on advanced breast cancer,compared with traditional paclitaxel.Methods Sixty-two cases with advanced breast cancer were randomly divided into test group(31 cases) given paclitaxel liposomal at 135 mg/m2 each session,and control group(31 cases) given traditional paclitaxel at 135 mg/m2 each session.Both groups received these regimens combined with ADM or EADM for two cycles(3 weeks per cycle).Results Of 62 cases,60 were eligible for the analysis of side effects and 60 for the overall response rate.The complete remission rate,partial remission rate and overall response rate were 10%,36.7%,46.7%in the test group and3.3%,40.0%,43.3%in the control group.There was no significant difference between the two groups.Though there was no significant difference in hematological toxicity between the two groups,the toxicity from the mixed solvent of polyethoxylated castor oil and ethanol was significantly lower in the test group than that in the control group.Conclusion Paclitaxel liposomal combined with ADM or EADM in the treatment of advanced breast cancer,being similarily effective as paclitaxel,can significantly lower the incidence of serious hypersensitive resctions.The rest of the toxicity of this two groups are similar.
