老年2型糖尿病并酮症患者应用胰岛素泵治疗的护理探讨

目的：探讨胰岛素泵在老年2型糖尿病酮症患者治疗中的应用及护理经验。方法：41例老年2型糖尿病酮症患者,应用胰岛素泵持续皮下输注胰岛素治疗19例（治疗组）,多次胰岛素皮下注射22例（对照组）,比较两组患者给予相应的全方位护理干预后的空腹血糖水平、血糖达标时间、酮症消除时间、住院时间和低血糖事件发生率。结果：治疗组低血糖事件发生率明显低于对照组（P〈0.01）,上述其他观察指标两组比较无显著性差异。结论：应用胰岛素泵持续皮下输注胰岛素治疗老年2型糖尿病酮症较多次胰岛素皮下注射更安全;在置泵前、置泵期间和置泵后不同时期给予相应的护理干预,有助于顺利完成治疗。

Nursing experience of CSII in elderly ketotic type 2 diabetic patients

Objective：To evaluate the appliciative and nursing experiences of subcutaneous insulin infusion（CSII） in elderly tpe 2 diabetic patients with ketosis.Methods：41 elderly ketotic type 2 diabetic patients were randomly divided into CSII group（n=19） and the multiply subcutaneous insulininjection group（n=22,control group）.The incidence of Hypoglcemia,the level of fasting blood glucose,the targeting-duration of blood glucose and the negative-duration of urine ketone bodies were compared between two groups.Results：The negative-duration of urine ketone bodise and the duration of targeting blood glucose were no difference between two groups,and the incidence of hypoglycemia was significantly lower in CSII group than that in control group（P0.01）.Conclusion：CSII is safer than multiply subcutaneous insulininjection in elderly type 2 diabetic patients with ketosis.The whole course and effective nursing is very important and helpful totreatment sucessfully.
