无偿献血者HBsAg ELISA筛查阳性结果确证分析

目的了解无偿献血乙肝筛查阳性样本确证实验结果 ,提高输血安全。方法采用进口和国产试剂酶联免疫吸附实验（ELISA）进行血液筛查,并以中和实验对阳性结果进行确证分析,评估试剂的灵敏度和特异性。结果 2009年郴州市无偿献血人群共计检出ELISA阳性387例（含进口或国产单一试剂检测阳性）,确证阳性331例,HBV阳性率为1.157%。进口试剂检出率1.157%,国产试剂0.877%,差异有统计学意义（χ2=79.01,P〈0.01）。结论有必要提高目前国产试剂的特异性和灵敏度;为减少输血后乙肝感染,血液筛查有必要采用高灵敏度、特异性较好的进口试剂。

Confirmation of Unpaid Blood Donors' Positive Results Screened Out by HBsAg ELISA

Objective To know the confirmatory test results of unpaid blood donors＇ positive samples in screening out hepatitis B, and to improve the safety of blood transfusion. Methods The blood samples were screened out by ELISA using import and domestic reagents, and the positive results were analyzed through neutralization test. The accuracy and idiosyncrasy of reagents were evaluated. Results Totally 387 positive cases among the blood donors in ChenzhOu in 2009 were detected through ELISA including import and domestic reagents tests respectively. 331 cases were confirmed positive, and the positive rate of HBV was 1.1.57%. The positive detection rates by using import reagents and domestic reagents were 1. 157% and 0.88？%, respectively, and the difference was statistically significant （x^2 = 79.01, P 〈0.01）. Conclusions It is necessary to improve the accuracy and idiosyncrasy of domestic reagents at present. The import reagents with high accuracy and fine idiosyncrasy are needed in screening out the blood in order to reducing hepatitis B infection after blood transfusion.
