脑血辨证之脑病证候与头风病证候对比分析

目的研究脑血辨证之脑病证候对头风病证候的适用性。方法采用统一的诊断标准，对每一入选病例根据脑病证候诊断标准和头风病证候诊断标准进行双重证候判别，并作配对分证，将配对分证结果列成双向无序R×C表，应用拟合优度疋。统计量进行独立性检验；应用KAPPA检验检验两种诊断标准的一致性，并计算证候符合率及灵敏度、特异度、假阴性率、假阳性率、似然比、符合率、正确诊断指数。结果两种辨证方法在证候分布方面互相独立，脑病证候与病情缓急、发病诱因、不良生活习惯、情志刺激、西医诊断病种具有明显的相关性。正确诊断指数为0．3731。Kappa值为0．3960。结论脑血辨证之脑病证候对头风病具有较好的适用性。脑病证候的分类建立于新病因理论的基础上，更加符合现代头风病病因病机的特点，具有较大的理论意义和实践价值，有利于头风病的辨证治疗。并且，脑血辨证规范化强，辨证思路清晰，易于掌握，错诊率低。

Contrast Analysis of Brain Illness Syndrome of the Brain Blood Syndrome Differentiation and Toufeng Syndrome

Objective To research in the serviceability of the brain illness syndrome of the brain blood syndrome differentiation to the Toufeng syndrome in TCM. Method Using the unified diagnosis standard, to analyze the dual syndromes of each selected case of illness according to the diagnosis standards of the brain illness syndrome and Toufeng syndrome in TCM and to make matches. By tabling the matched results in disorderly tow-direction R× C to checkout the independences of them using the statistic goodness of fit χ2, and to examine the coherence of two kinds of diagnosis standards using KAPPA. And the coincidence rate, the meter sensitivity, the peculiarity, the false negative rate, the false positive rate, the likelihood ratio, the correct diagnosis index were calculated. Result Two dialectical methods were mutually independent in the syndrome distribution. Brain illness syndrome was obviously connected with the condition emergency, disease causes, bad habits and customs, the sentiment stimulation, and western medicine diagnose. The correct diagnosis index was 0.373 1 and the Kappa was 0.396 0. Conclusion The brain illness syndrome of the brain blood syndrome differentiation has a good serviceability to Toufeng syndrome in TCM. Furthermore, the classification of brain disease, which is established on the new theory of etiology, more conforms to the characters of the disease cause and pathogenesis of Toufeng syndrome in TCM, and has a great theoretical significance and practical value. It is advantageous to the treatment differentiation based on syndrome of Toufeng syndrome. And moreover, the brain blood syndrome differentiation is more standardized with clear thought and easy to grasp, and makes a lower rate of wrong diagnosis.
