妊娠合并上尿路结石的诊断和治疗

目的探讨治疗妊娠合并上尿路结石的有效方法。方法对8例妊娠合并上尿路结石患者的临床资料及诊治经过进行回顾性分析,结合文献讨论本病的临床特点和治疗方法。结果8例患者均有腰痛、一侧腰腹部压痛和肾区叩击痛、肉眼或镜下血尿。8例患者中依据临床表现诊断2例,B超诊断5例,腹部X线平片诊断1例。6例经保守治疗结石自行排出,2例分别放置双J管及经皮肾造瘘管,于产后行体外冲击波碎石(ESWL)治愈。早产1例,无流产和胎儿畸形。结论应针对性选择对各期妊娠影响小的方法诊治本病以减少并发症。

The Diagnosis and Treatment of the Upper Urinary Tract Calculi Complicating Pregnancy

Objective To evaluate the diagnosis and treatment of the upper urinary tract calculi complicating pregnancy.Methods A retrospective study was made on 8 cases clinical data of the upper urinary tract calculi complicating pregnancy of the presenting symptoms,diagnosis and treatment,in combination with related literature review.Results All cases had flank pain,tendeness and percussion pain on real region,micro or macroscopic hematuria.2 cases were dignosed by the clinical features,5 cases by ultrasonography,1 case by plain KUB film.With the conservative management,spontaneous passing of stones was noted in 6 cases,double J stents were placed in one case and percutaneous nephrostomy was done in another case and extracorporeal shock-wave lithotripsy(ESWL) was performed successfully in the post partum period.Preterm birth occurred in 1 case and no abortion or congenital anomalies of the infant was noted.Conclusion The diagnosis and treatment of the upper urinary tract calculi complicating pregnancy with the less impact on pregnancy methods is safe and effective,may be helpful to reduce morbidity.
