七氟醚或异氟醚对神经外科患者脑氧代谢的影响

摘    要:目的 观察1.5 MAC七氟醚和相同浓度异氟醚联合靶控持续输注瑞芬太尼对神经外科患者术中颈静脉球氧饱和度(SjvO2)、脑动静脉血氧分压(Da-jvO2)、脑氧摄取率(COER)的影响.方法 采用随机、双盲、对照的方法,将50例幕上占位患者随机分为S组,I组,每组25例于切开硬膜即刻(T0),开硬膜1 h后(T1)、2 h后(T2)以及关硬膜即刻(T3)各个时点,同时采集桡动脉,颈内静脉球血行血气分析 测定SjvO2并计算Da-jvO2及COER值.结果 S组T1、T2、T3各个时点的SjvO2值均较T0升高(P〈0.05或〈0.01),而AjvDO2值、COER值均较T0明显降低(P〈0.05或〈0.01)S组在T1、T2、T3时点的SjvO2值较I组相应时点明显升高而AjvDO2值、COER值明显降低(P〈0.05).结论 神经外科手术中1.5 MAC七氟醚较同浓度异氟醚,能有效降低脑氧代谢,起到一定的脑保护作用.
