慢性胃炎患者胃黏膜环氧合酶-2的表达

摘    要:目的 探讨环氧合酶 (COX - 2 )表达与胃黏膜病变的相关性。方法 胃黏膜标本共 1 2 0例 ,采用免疫组织化学技术对胃黏膜COX - 2蛋白表达进行检测。结果 胃窦黏膜表面上皮和固有腺上皮的胞浆中 ,均可见到COX - 2蛋白表达。COX - 2蛋白表达在不同性别、年龄的患者间 ,差异无显著性。不同胃黏膜病变COX - 2蛋白表达阳性百分率以不典型增生最高 (90 % ) ,其次为肠组织转化 (80 % )、慢性萎缩性胃炎(5 0 % ) ,其差异有显著性 (P <0 0 0 1 )。在COX - 2蛋白强阳性表达中 ,以胃癌前病变 (不典型增生和肠组织转化 )为主 ,占 72 2 % ;而在COX - 2蛋白表达阴性中 ,以慢性浅表性胃炎和慢性萎缩性胃炎为主 ,2者合计占78 1 %。结论 COX - 2表达强度与胃黏膜病变有明显的相关性 ,胃癌前病变COX - 2蛋白表达明显增高

Abstract:Objective To investigate the expression of COX-2 protein in gastric epithelial cells. Methods Gastric biopsy from 120 subjects was examined immunohistochemically for COX-2 protein expression. Results COX-2 protein expression localized to the cytoplasmic region of foveolar and glandular epithelia with variable levels in the epithelium. There was no correlation between COX-2 protein expression and the age or gender of patients. The positive rate of COX-2 protein expression in antral was significantly higher in patient with dysplasia than that in intestinal metaplasia, chronic atrophic gastritis and chronic superficial gastritis (90% vs 80%, 50%, 43.3%, respectively (P<0.01). COX-2 protein expression was significantly higher in patients with gastric precancerous conditions than non-precancerous conditions (85% vs 46.7%, P<0.001). The positive rate of COX-2 protein expression was positive correlation with gastric histopathology (r=0.409, P<0.001). Conclusion There was positive correlation betwwen histopathology and COX-2 protein expression. COS-2 protein expression was significantly higher in patients with gastric precancerous conditions than that of non-precancerous conditions.
