对于重组人脑利钠肽治疗老年慢性阻塞性肺病心衰患者的观察

目的：观察重组人脑利钠肽（rhBNP）治疗老年慢性阻塞性肺病心衰患者加重期的临床效果。方法：90例年龄≥65岁的慢性阻塞性肺病心衰加重期患者被随机分为rhBNP组（44例）和硝酸甘油治疗组（46例），观察两组患者治疗前后的临床疗效、心功能及血清BNP水平的变化。结果：与硝酸甘油组比较，rhBNP治疗组治疗后24h临床显效率（21．7％比45．5％）及总有效率（54．3％比91．0％）明显升高（P均〈0．05）；动脉血氧分压（9．44±1．41）kPa比（12．01±1．65）kPa]明显升高（P〈0．05）。治疗1周后，与硝酸甘油组比较，rhBNP组左室射血分数明显升高（43±7）％比（48±8）％]，P〈0．05。结论：重组人脑利钠肽治疗慢性阻塞性肺病心衰患者加重期安全有效。

Effect of recombinant human brain natriuretic peptide in aged patients with exacerbation of chronic obstructive pulmonary disease and heart failure

Objective： To observe effect of recombinant human brain natriuretic peptide （rhBNP） in aged patients with exacerbation of chronic obstructive pulmonary disease （COPD） and heart failure （HF）. Methods： A total of 90 patients （965 years） with exacerbation of COPD and HF were randomly divided into rhBNP group （n = 44） and ni- troglycerin group （n = 46）. Clinical curative effect and changes of cardiac function and serum BNP level were ob- served in two groups before and after treatment. Results： Compared with nitroglycerin group, there were significant increase in markedly effective rate （21.7% vs. 45.5%）, total effective rate （54. 3% vs. 91.0%） and PaO2 （9.44 ± 1.41） kPa vs. （12.01 ± 1.65） kPa] in rhBNP group 24 h after treatment, P〈0.05 all; compared with nitro- glycerin group one week after treatment, there was significant increase in left ventricular ejection fraction （43 ± 7） % vs. （48 ±8）%1, P〈0. 05. Conclusion： Recombinant human brain natriuretic peptide is safe and effective in treatment of patients with exacerbation of chronic obstructive pulmonary disease and heart failure.
