骶前纵韧带拉力及静脉注水试验对治疗骶前静脉丛破裂的临床意义

经尸体骶前韧带拉力及谫静脉丛注水试验及临床实践，探讨ＰＳＶＰ大出血的治疗。方法测量３４具成人尸体骶前纵韧带厚度，并行拉力试验。对２０具成人尸体ＰＳＶＰ采用与血管走行方向垂直缝扎ＰＳＶＰ断裂各端，网膜填塞压迫骶椎前穿通支静脉开口或人为骨性通道，将骨膜缝扎在骶前纵韧带上，再向破裂处注水，观察水从缝扎外流出情况。

Clinical significance of presacral vertical liagment pulling force and injection of water into ruptured presacral venous plexus

Objective To treat the massive bleeding of presacral venous plexus(PSVP) by experiment of the presacral vertical liagment pulling force and injecting water into PSVP. Methods The presacral vertical liagment were measured and the pulling force were experimented in 34 cadavers. Ends of the ruptured PSVP were stitched and ligated through a vertical way to the veins. The vein break and the artificial bony passageways were penetrated by the sacral basivertebral which was filled and pressed with free omentum.The omentum was stiched on the presacral vertical ligament in 20 cadavers. Then, water was injected into ruptured PSVP. Water flowing from sutured area was observed. Such methods were used to stop massive bleeding of PSVP in 4 cases in clinic. Results The presacral vertical ligament was able to support 5 2 kg pulling force. There was no water flowing out from sutured area even under a pressure over 1 000 mm H 2O (1 mm H 2O=0.0098 kPa). No bleeding recurred in 4 cases. Conclusions Above mentioned measures are effective to stop water flowing out from sutured area and to stop massive bleeding of PSVP in clinic.
