免疫学指标联合检测对类风湿关节炎的诊断价值

目的探讨免疫学指标类风湿因子（RF）、抗环瓜氨酸多肽抗体（抗CCP）和抗角蛋白抗体（AKA），免疫球蛋白G（IgG）、C3、C4在类风湿关节炎（RA）诊断中的临床应用价值。方法对120例RA患者（RA组）、130例其他自身免疫系统疾病的患者（非RA组）、60例健康人（对照组）同时进行RF，抗CCP，AKA，IgG，C3、C4检测。结果RA组抗CCP、RF、AKA的阳性率均明显高于非RA组和对照组（P〈0．05）；RA组IgG水平较对照组明显升高（P〈0．05）；RA组C3、C4水平与对照组比较，差异无统计学意义（P〉0．05）。结论RA患者体内存在多种免疫功能紊乱，联合检测免疫学指标对疾病的诊断、治疗和预后有重要的意义。

The diagnosis value of combined detection of immunological indicators for rheumatoid arthritis

Objective To explore the clinical application value of the immunological indicators：rheumatoid factor （RF）, anti-cyclic citrullinated peptide antibody （anti-CCP）,anti-keratin antibodies （AKA）,immunoglobulin G （IgG）,C3 and C4 in the diagnosis of rheumatoid arthritis. Methods RF,anti-CCP,AKA,IgG,C3 and C4 of blood serum were detected in 120 patients with rheumatoid arthritis （RA）,130 patients with other autoimmune diseases and 60 cases of healthy people selected as normal control group. Results The positive rate of anti-CCP, RF and AKA in the RA group was significantly higher than that in the non-RA group and the normal control group （P〈0.05）; IgG of RA group was sig- nificantly higher than that in the normal control group （P〈0.05）;C3 and C4 of RA group was lower than that in the normal control group,but had no statistical difference （P〉0.05）. Conclusion A variety of immune function disorders exist in patients with RA in vivo,it has important significance for diagnosis,treatment and prognosis of the disease with combined detection of immunologicalindicators.
