腹腔镜治疗早期宫颈癌的临床效果分析

目的 分析腹腔镜下广泛子宫切除术＋盆腔淋巴结清扫术治疗早期宫颈癌的临床效果.方法 将本院2009年1月～2012年3月收治的134例宫颈癌早期患者随机分为两组,其中观察组73例行腹腔镜下广泛子宫切除术＋盆腔淋巴结清扫术,对照组61例行开腹广泛子宫切除术＋盆腔淋巴结清扫术,比较两组患者的相关临床指标及术后并发症发生情况.结果 观察组患者手术时间为（179.1±35.9） min、术中出血量为（109.5±33.1） ml、术后肛门排气时间为（1.5±0.3）d、住院天数为（11.7±1.2）d,均低于对照组的（298.4±43.5） min、（305.2±38.6） ml、（2.6±0.7）d、（18.3±1.6）d;总并发症发生率为9.6％,明显低于对照组的24.6％,差异有统计学意义（P＜0.05）.结论 采用腹腔镜下手术治疗早期宫颈癌,具有创伤小、手术时间短、术中出血量少、住院时间短及术后并发症少等优点,值得临床推广实施.

Analysis of clinical effectiveness of laparoscopic surgery for early cervical cancer

Objective To analyze the clinical effectiveness of laparoscopic extensive hysterectomy plus pelvic lymph node cleaning for early cervical cancer. Methods One hundred and thirty four patients with early cervical cancer ad- mitted to our hospital from January 2009 to March 2009 were randomly assigned into two groups,that was observation group of 73 patients who underwent laparoseopie extensive hysterectomy plus pelvic lymph node cleaning and control group of 61 patients who underwent open surgery of extensive hysterectomy plus pelvic lymph node cleaning.The relat- ed clinical indicators and postoperative complications of patients in two groups were compared. Results The operation time,amount of bleeding during operation, postoperative time of anal aerofluxus,and hospital stay of patients in obser- vation group, respectively （179.1±35.9） min,（109.5±33.1） ml,（l.5±0.3） d,and （11.7±1.2） d,were less or shorter than those of control group,（298.4±43.5） min,（43.5±38.6） m1,（2.6±0.7） d,and （18.3±1.6） d,respectively.The overall incidence of complications of observation group （9.6%） was also significantly lower than that of eontrol group （24.6%） with statistical difference （P〈0.05）. Conclusion The laparoscopic surgery for early cervical cancer has advantages of mild trau- ma, less operation time,less bleeding during operation,shorter hospital stay, and less postoperative complications,also is worth widely clinical promotion and applying.
