摘    要:①目的 观察早期康复训练对大鼠大脑中动脉缺血再灌注后功能恢复的影响 ,并探讨其分子机制。②方法 应用线栓法建立Wistar大鼠大脑中动脉闭塞再灌注模型 (MCAO) ,随机分为康复治疗组、自然恢复组和假手术组。以原位杂交及图像分析技术检测再灌注后 6h、1d、3d、7d、1 4d、2 1d缺血半影区生长相关蛋白 (GAP 4 3)和突触素 (P38)mRNA的表达。③结果 脑缺血后缺血半影区GAP 4 3和P38mRNA表达分别自术后 6h、3d开始增高 ,第 7天达到高峰 ,自第 1 4天开始降低。康复治疗组缺血半影区GAP 4 3和P38mRNA的表达在各时间点均高于自然恢复组 ,仅在 1 4d时有显著性差异 (t=4 .1 3、4 .6 2 ,P <0 .0 5 )。④结论 早期康复运动可促进卒中后的神经功能恢复 ,其分子机制可能与提高缺血半影区GAP 4 3和P38mRNA的表达有关

Abstract:Objective\ To observe the effects of early rehabilitation training on the functional outcome in rats after reperfusion for occlusion of middle artery in brain and explore its molecular mechanism.\ Methods\ Fifty two Wistar rats were subjected to 60 minutes of middle cerebral artery occlusion (MCAO) and different time of reperfusion and divided into physical training group,nature recovery group and sham operated group. The expressions of mRNA including GAP 43 and P38 associated with neurite plasticity were determined by in situ hybridization. \ Results\ The GAP 43 and P38 mRNA expressed as early as 6 h and 3 d, and reached to the peak on day 7, and came down from day 14. The GAP 43 and P38 mRNA expressed remarkably on day 14 in ischemic penumbra in physical training group. The significance of difference showed only on day 14 ( t=4.13, 4.62, P <0.05). Conclusion\ The early physical training could improve behavioral outcome of MCAO rats. It might be correlated with the elevation of the expressions of GAP 43 and P38 mRNA in ischemic penumbra.
