N末端B型利钠肽原在急诊老年呼吸困难患者鉴别中的应用意义

目的 研究血浆N末端B型利钠肽原(NT-proBNP)水平对急诊老年呼吸困难患者鉴别诊断的临床意义.方法 急诊科就诊老年呼吸困难患者182例,分为心源性呼吸困难和肺源性呼吸困难两组.分别检查血浆NT-proBNP含量,并给予心脏超声检查,比较两组患者NT-proBNP含量和左室射血分数的区别;并对心源性呼吸困难组NT-proBNP含量与左室射血分数进行相关分析.结果 肺源性呼吸困难组NT-proBNP含量为108.5±25.7 pg/ml,心源性呼吸困难组NT-proBNP含量为945.1±143.8 pg/ml(P=0.001);相关分析显示老年心源性呼吸困难组NT-proBNP与左室射血分数呈负相关关系(r=-0.62,P<0.01).结论 NT-proBNP对老年呼吸困难患者鉴别诊断具有一定临床意义.

Use of amino-terminal portion of brain natriuretic peptide to identify causes in emergency elderly patients with dyspnea

Objective To study clinical significance of amino-terminal portion of brain natriuretic peptide levels in elderly patients emergency with dyspnea.Methods 182 elderly patients with dyspnea were divided into cardiogenic dyspnea and pulmonary dyspnea group.Plasma NT-proBNP content and cardiac ultrasound doppler were examined and compared between the 2 groups,then the connection between the content of NT-proBNP and left ventricular ejection fraction correlation was analyzed.Results It contained 108.5±25.7 pg/ml NT-proBNP in the pulmonary dyspnea group and 945.1±143.8 pg/ml in the cardiogenic dyspnea group(P=0.001).Correlation analysis showed that NT-proBNP and left ventricular ejection fraction was negatively correlated in the elderly cardiogenic dyspnea group(r=-0.62,P<0.01).Conclusion NT-proBNP has certain clinical significance for the differential diagnosis in elder patients with dyspnea.
