肠内营养液的个体化应用

摘    要:目的介绍一种经济简单的肠内营养液的个体化配制方法及其临床应用。方法利用商品化的肠内营养制品为主要原料,根据患者病情,再加入氨基酸、葡萄糖、食用植物油等基本营养元素,以乳化技术个体化配制而成。随机将89例外科住院患者分为肠内营养支持组(49例)和膳食组(40例),分别给予自制的肠内营养液和家属自制食物,比较两组患者术后15天的伤口愈合率、淋巴细胞计数和血清白蛋白;同时另将47例因营养不良需采用肠内营养支持的住院患者,根据病情个体化分别给予白配肠内营养液(27例)和商品肠内营养乳剂(20例),分别观察营养支持10天后其转甲状腺素及白蛋白水平变化以及两组成本比较。结果自制肠内营养液符合口服乳剂的各项质量指标。与普通膳食比较能较快促进伤口愈合率和提高淋巴细胞计数(P〈0.05);而与商品化的肠内营养液比较,在达到相同的肠内营养支持效果的基础上,个体化配制的肠内营养液更为经济(P〈0.05)。结论个体化配制肠内营养液不但经济,而且能更灵活应用于各种病情下患者的营养支持。

Abstract:Objective To describe the individualized preparation and clinical application of a simple and cost-effective enteral nutrition liquid.Methods With the commercially available enteral nutrition liquid as the raw material,the individualized product was prepared,according to the patient's health condition,by adding amino acids,glucose,edible vegetable oil,and other essential nutrition elements.Totally 89 surgical inpatients were enrolled and randomly divided into the enteral nutrition support group(n=49)and the normal diet group(n=40),and their clinical outcomes,including wound recovery rate,lymphocyte count,and serum albumin levels,were observed at postoperative 15 days.Another 47 hospitalized patients with malnutrition were also enrolled in this study,among whom the individualized enteral nutrition liquid(n=27)and commercially available products(n=20)were given according to their nutrition conditions.The changes of thyroxin and albumin levels as well as the cost-effectiveness between these two groups were compared 10 days later.Results The homemade enteral nutrition liquid met the requirements of orally-administered emulsion,and significantly promoted the quick recovery of wounds and increased the lymphocyte count compared with normal diet(P<0.05).When compared with the commercially available enteral nutrition liquid,it provided a more cost-effective solution(P<0.05).Conclusion The individualized enteral nutrition liquid is a feasible solution for hospitalized surgical patients with different health conditions.
