鼻咽癌放射治疗患者口咽部真菌感染分析

目的 调查分析鼻咽癌患者放疗过程中并发口咽部真菌感染及相关因素，探讨行之有效的预防对策，指导临床护理。方法 对1999～2001年鼻咽癌行放射治疗住院患者的出院病历进行统计学分析。结果 128例中经咽拭子培养证实，发生口咽部真菌感染31例，占24．2％；检出致病真菌主要为白色念珠菌和热带念珠菌；鼻咽癌放射治疗并口咽部真菌感染与肿瘤自身免疫功能低下、放疗导致放射性口腔粘膜损伤、机体屏障破坏、不合理应用抗生素和糖皮质激素等因素有关，并且随着住院时间延长，真菌感染发生率增加。结论 应明确预防感染的重要性、做好消毒隔离工作、改善病房环境、合理应用抗生素和激素。

Nasopharyngeal Carcinoma Combined with Oropharynx Fungus Infection During Radiotherapy: Analysis and Prevention

OBJECTIVE To investigate and analyse the nasopharyngeal carcinoma patients combined with oropharynx fungus infection during radiotherapy and its relative factors. METHODS Medical records of nasopharyngeal carcinoma patients from 1999 to 2001 in our hospital were statistically analysed. RESULTS There were 31 nasopharyngeal carcinoma patients combined with oropharynx fungus infection from 128 cases, the infection rate was 24.2%. The fungus detected primarily was Candida albicans and C. tropicalis. Nasopharyngeal carcinoma patients combined with oropharynx fungus infection during radiotherapy were relative to low tumor autoimmunity function, oral mucosal lesion resulted in radiotherapy, destruction to body barrier, improper application of antibiotic and glucocorticoids and so on. CONCLUSIONS The significance of prevention, sterilization and isolation, changing the environment of ward, rational using antibiotics and glucocorticoids should be emphasized.
