动态房角镜下周边浅前房患者虹膜双峰征与高褶虹膜形态的相关性研究

摘    要:目的 探讨动态房角镜下周边浅前房患者虹膜双峰征与高褶虹膜形态的关系.方法 收集2012年6月至2013年3月在北京安贞医院眼科门诊就诊、经裂隙灯检查确诊为周边浅前房患者74例,对74只周边浅前房患眼进行动态检查,比较不同类型浅前房患眼中虹膜双峰征的阳性率.结果 根据检查结果,瞳孔阻滞型31例,睫状突前旋型14例,虹膜肥厚型7例,混合型22例.睫状突前旋型患者阳性率为92.3% (13/14),明显高于瞳孔阻滞型和虹膜肥厚型(14.3%,1/7) (P <0.01).在混合型患者中,经激光虹膜周边切除术去除瞳孔阻滞因素后,虹膜双峰征的阳性率在存在睫状突前旋型患者中仍明显高于虹膜肥厚型的患者[86.7%(13/15)比14.3% (1/7),P<0.01].结论 虹膜双峰征是预测睫状体前旋型浅前房的有效体征.

Abstract:Objective To investigate the relationship between iris double hump with indentation of gonioscope and the plateu iris configuration with ultrasonic biological microscope.Methods A prospective study was conducted to observe 74 patients with shadow anterior chamber.The positive incidence among groups of patients caused by different types were compared.Results Iris double hump was positive in 92.3% (13/14) patients with ciliary process rotation forward,which was much higher than in pupilary block (0.0%) and iridauxesis groups (14.3%,P 〈 0.01).In patients with combined mechanisms,after laser iridectomy,iris double hump was positive in 86.7% (13/15) patients with ciliary process rotation,which was also higher than that in iridauxesis group (14.3%,P 〈 0.01).Conclusion Iris double hump is a good prognosis for ciliary process rotation.
