胫前大疱性表皮松解症并发落叶型天疱疮1例

报告胫前大疱性表皮松解症并发落叶型天疱疮1例。患者男，64岁。全身红斑、瘙痒1年，出现松弛性水疱、糜烂、结痂5个月。患者自10岁始四肢出现瘙痒性疱疹，渐形成红色瘢痕，并持续至今。皮损组织病理检查：大疱处示棘层上部大疱，疱中有嗜酸性纤维蛋白网、大量中性粒细胞及棘层松解细胞；胫前处皮损示表皮下裂隙性水疱，较多成纤维细胞和结缔组织。间接免疫荧光检查抗表皮细胞间基质抗体（＋），滴度为1：40。

A case of pretibial epidermolysis bullosa with pemphigus foliaceus

A case of pretibial epidermolysis bullosa with pemphigus foliaceus is reported.A 64-year-old male presented with generalized erythema for 1 year,and flaccid bullae,erosions,crusts for 5 months.When he is 10 years old the itching blisters appeared on his extremities and then the blisters became gradually albopapuloid.In the bullae area the histological examination showed acantholysis in the upper epidermis,a formation of eosinophilic spongiosis and neutrophiles infiltration.Acantholytic cells were observed.In the anterior shin area a noninflammatory subepidermal bullae and a hyperplastic connective tissue of dermis were present.Indirect immunofluorescent test showed that the intercellular antibody was positive (1:40).
