剖宫产史合并前置胎盘对母儿围生期的影响

目的探讨有剖宫产史的前置胎盘围产期并发症。方法回顾性分析该院产科分娩病例中前置胎盘合并剖宫产史与前置胎盘行初次剖宫产的产妇的产前出血、产后出血、胎盘植入率、子宫切除率及新生儿结局。结果两组的平均产前出血量、产后出血发生率无差异（P〈0.05）。但有剖宫产史组的胎盘植入率、子宫切除率、平均产后出血量及产后大量出血发生率、新生儿窒息率及早产率均明显高于前置胎盘初次剖宫产史组（P〈0.05）,并且平均出生体重明显低于后者（P〈0.05）。结论前置胎盘对母儿的生命安全存在严重威胁,其中剖宫产史合并前置胎盘对母儿的危害更大,需引起高度重视。

Influence on the perinatal patients who had placenta previa with previous cesarean section

Objective To investigate the perinatal complications of the patients with placenta previa who had the history of cesarean section. Methods We retrospectively analyzed the placenta previa cases in our hospital. Placenta previa complicated with and without cesarean section were compared by perinatal complications and neonatal endings. Results The placenta previa complicated with previous cesarean section had higher risks of placenta accreta,hysterectomy,heavy bleeding,asphyxia,low birth weight and premature（ P ＆lt; 0. 05）. Conclusions Placenta previa will threaten the lives of mothers and children. The patients who had the history of cesarean section are more vulnerable,to which we need to pay close attention.
