L—精氨酸对高肺血流量所致肺动脉高压的防治作用

摘    要:目的 探讨L-精氨酸对高肺血流量所致肺动脉高压的防治作用。方法 21只雄性SD大鼠随机分为对照组(n=6)、分流组(n=7)和分流+L-精氨酸组(n=8)。对分流组和分流+L-精氨酸组大鼠行腹主动脉-下腔静脉分流术。11周后以右心导管法测定肺动脉平均压(mPAP)。检测右心室/体重(RV/BW)和右心室/左心室+室间隔(RV/LV S)比值。并且观测肺血管结构的变化。结果 分流组大鼠mPAP、RV/BW及RV/(LV S)比值明显高于对照组(P均<0.01),且分流组大鼠肺小血管肌化程度明显增强,肺中、小肌型动脉相对中膜面积及厚度明显增加。而分流+L-精氨酸组大鼠mPAP、RV/BW及RV/LV+S比值明显低于分流组(P均<0.05),L-精氨酸缓解了肺血管结构重建的形成。结论 L-精氨酸对高肺血流量所致肺血管结构重建及肺动脉高压有重要的调节作用。

Abstract:Objective To explore the therapeutic effect of L-arginine on high pulmonary blood flow-induced pulmonary hypertension.Methods 21 male SD rats were randomly divided into shunting group, shunting with L-arginine group, and control group. Abdominal aorta and inferior vena cava shunting were produced in rats of shunting group and shunting with L-arginine group. After 11-week shunting, pulmonary artery mean pressure ( mPAP ) of each rat was evaluated by using a right cardiac catheterization procedure. The ratio of right ventricular mass to body weight (RV/BW) and the ratio of right ventricular mass to left ventricular plus septal mass (RV/LV S) were detected. Pulmonary vascular microstructure was measured.Results mPAP, RV/BW and RV/LV S were significantly increased in shunt rats as compared with those of normal controls(P<0.01, respectively). Muscularization of small pulmonary artery and relative medial thickness and relative medial area of pulmonary muscularized arteries were obviously increased in shunt rats compared with controls. However, mPAP, RV/BW and RV/LV S were significantly decreased in rats of shunting with L-arginine group as compared with shunt rats (P<0.05, respectively).L-arginine ameliorated pulmonary vascular structural remode-ling.Conclusion L-arginine plays an important role in regulation of the development of high pulmonary blood flow-induced pulmonary vascular remodeling and pulmonary hypertension.
