腹腔镜超声在妇科手术中的应用

目的　探讨腹腔镜超声在妇科手术中的应用价值。方法　应用腹腔镜超声检查７例患者盆腔病变。结果　①宫内节育器残留宫腔伴下腹痛１例， 经宫腔镜取出残环； ②原发不孕伴子宫内膜息肉１例，超声监导下切除息肉；③胚胎残留宫腔， 多次清宫后致宫腔粘连及高强回声灶１例， 在超声监视下切除粘连组织；④疑胎骨残留宫腔１例， 在超声监视下切除病变组织； ⑤右输卵管扩张、积水， 与盆腔粘连１例，手术解除粘连， 切除右侧输卵管；⑥双侧输卵管粘连、末端扩张积液１例， 手术切开末端， 放出积液；⑦双附件肿物１例（一侧为单纯囊肿， 一侧为畸胎瘤），行手术切除。结论　腹腔镜超声在监视复杂的宫腔镜手术及腹腔镜手术前的定位诊断上有独到的应用价值。

Laparoscopic ultrasonography using in gynecology operation

Ohjective To evaluate the using of laparoscopic ultrasonography in gynecology operation.Methods Laparoscopie ultrasonography was performed in 7 patients with pelvic diseases. Results ①Intrauteringe device was remained in the uterine cavity with hypogastralgia and the device was removed with operating hysteroscope.②Primary sterility with uterine polyp which was resected under ultrasonography monitoring.③Embryo was left over the repeated cavity,which the repeated uterine curettages result in the caity adhesion with focal strong echo and the adhesion was resected with the ultrasonographic guidance.④It was suspected that the fetal bone was stay at the cavity,the lesion was resected by the ukrasonographic monitoring.⑤The right fallopian tubal dilatation,hydrosalpinx was misdiagnosed the right oophoritic cyst.⑥Bilateral end of fallopian tubes dilatation and hydrosalpinx.⑦Bilateral oophoroma,one of which was oophoritic cyst and another was dermoid cyst of vary,both of them were resected by laparoscopie ukrasonography, Conclusions It has origingal value that the laparoscopm ultrasonography monitor the complex hysteroseopic operation and the disease is located before laparoscopic operation.
