肝门部胆管癌的超声影像诊断

目的　探讨肝门部胆管癌的超声影像特点及早期诊断的临床价值。方法　对１２例经病理学证实为肝门部胆管癌患者的临床及超声影像资料进行回顾性分析。结果　超声直接征象分为乳头型（１例）、团块型（５例）、截断型（４例）和狭窄型（２例）四型， 并可显示肝内胆管扩张、肝叶萎缩及肝门肿大淋巴结等间接征象。结论　超声显像对肝门部胆管癌的诊断及鉴别诊断有重要价值， 早期诊断应得到进一步重视

Ultrasonographic diagnosis of hilar cholangiocarcinoma

Objective\ To investigate the characteristics of ultrasonography of hilar cholangiocarcinoma and to explore its clinical value in early diagnosis. Methods\ The sonographic findings and clinical appearance of 12 patients with pathological proof of hilar cholangiocarcinoma were retrospectively analysed. Results\ Ultrasonic features of hilar cholangiocarcinoma was divided into 4 types: papilla (1 case), round mass (5 cases), abruption (4 cases) and stenosis (2 cases). On sonography, other signs could be observed such as dilated intrahepatic bile ducts, intraductal mass at the confluence of the right and left intrahepatic ducts, enlarged portal lymph nodes, and lobar atrophy. Conclusions\ The ultrasonography has higher diagnostic and differential value for hilar cholangiocarcinoma, and the application of early diagnosis should be further explored.
