实时B超引导下经皮跖腱膜切断术:尸体足标本研究

摘    要:目的探讨实时B超引导下经皮跖腱膜切断术治疗跖腱膜炎/跟骨骨刺综合症的可行性。方法在实时B超引导下对8只新鲜尸体足标本进行经皮跖腱膜切断术。术后行解剖观察。结果通过解剖观察发现所有标本中跖腱膜均按预定的设想被完全切断。跖腱膜深面的趾短屈肌损伤深度小于2mm,其余肌肉及足底内外侧动、静脉和神经均无损伤。结论这一新术式具有操作简便、安全准确、组织损伤小、可重复性好等优点,可进行临床实际应用

Abstract:Objective To explore the feasibility of percutaneous planlar fasciotomy guided by real time ultrasound for treatment of plantar fasciitis/heel spur syndrome. Methods Percutaneous planlar fasciotomies guided by ultrasound were performed of eight fresh cadaveric feet and then anatomic observations were carried out. Results It was found that all planlar fascias in cadaveric specimens were thoroughly severed according to predetermined plan. Damages to musculus flexor digitorum brevis underneath the plantar fascia were less than 2 mm. And the other muscles, medial and lateral plantar arteries, veins and nerves were not involved. Conclusions This new procedure has the advantages of simplicity, safety, accuracy, minimal damages to surrounding tissues and high reproductivity. Therefore, the authors suggest that it can be clinically used in management of plantar fasciitis/heel spur syndrome.
