高频及彩色多普勒血流显像对睾丸及附睾疾病鉴别诊断的价值

摘    要:为探讨高频及彩色多普勒血流显像(CDFI)对睾丸、附睾及精索疾病诊断及鉴别诊断的价值,采用高频及CDFI观察睾丸、附睾及精索病变的二维声像图、血流状况以及频谱多普勒血流参数。结果显示:(1)CDFI显示睾丸内部丰富血流主要鉴别睾丸炎、睾丸肿瘤及睾丸扭转的复位。睾丸炎动脉静脉伴行,走行规则;睾丸肿瘤动脉分支增多、走行不规则;睾丸扭转复位者血流由无到多的过程明显;(2)睾丸内部无血流或明显减少者主要见于睾丸扭转,睾丸炎坏死及睾丸外伤。(3)CDFI显示附睾的短线短或斑点状血流符合附睾炎诊断并对附睾结核的鉴别具有重要价值,附睾淋巴管瘤血流不丰富。提示:CDFI的应用对于睾丸炎、睾丸肿瘤及睾丸扭转的鉴别诊断及附鼻炎的诊断具有重要价值。

Abstract:The value of high frequency and CDFU in the differential diagnosis of testicle and epididymis diseases was evaluated. High frequency and color Doppler flow imaging (CDFI) were applied to observe the dimensional imaging of testicle and epididymis, blood flow and Doppler blood flow parameters in 72 cases of testicle and epididymis diseases. Results showed: (1) CDFI showing affluent blood flow inside testicle might be a must for differential diagnosis of testitis, testicle tumor and torsive testicle reposition; (2) No blood flow or obviously decreased blood flow inside testicle observed in testicle torsive, testitis and testicle trauma; (3) CDFI showed short line or mottling blood flow inside epididymis. It was concluded that CDFI played an important role in the differential diagnosis of testitis, testicle tumor, testicle torsion and epididymitis.
