急性超容性血液稀释用于骨科出血较多病人的临床研究

[目的 ]探讨急性超容性血液稀释技术 (AHHD)用于骨科出血较多病人的有效性和安全性。[方法 ]将 2 0例择期骨科手术病人随机分为对照组与实验组。实验组在术前应用AHHD ,观察AHHD对病人血流动力学、血流变及凝血功能的影响。 [结果 ]实验组的血液动力学较对照组稳定 ;血液稀释后的血液粘稠度下降 ,红细胞比容下降 ,但术后两组无差异 (P >0 .0 5 ) ;凝血功能两组无差异 (P >0 .0 5 ) ;术后的输血量实验组少于对照组。 [结论 ]AHHD可安全的应用于骨科出血多的手术 ,并可提高病人对失血的耐受性 ,减少异体血输入量

To explore the effectiveness and safety of applying acute hypervolemic hemodilution (AHHD) technique on orthopaedic patients with substantial hemorrhage. Divide 20 patients who were to receive surgical operations on a set date randomly into a comparison group and an experiment group. Apply AHHD on patients in the experiment group before surgical operations and observe the influence of AHHD on the patients' hemodynamics, hemorheology and thrombosis ability. The hemodynamics of patients in the experiment group were stable than that of the comparison group; After blood dilution, blood viscosity drops, so was hematokrit, although there were no differences after surgeries; No differences were identified in terms of thromboses ability; After surgical operation, the volume of blood transfusion in patients in the experiment group was less than hat of the comparison group.[Conclusion]AHHD can be applied safely on orthopaedic patients with substantial hemorrhage, and it can also enhance the endurance of patients to the loss of blood and thus reduces the volume of cross blood transfusion.
