目的探讨关节腔灌洗术治疗早期颞下颌关节不可复性盘前移位临床疗效。方法对2006年5月至2007年5月鞍山市口腔医院口腔颌面外科收治的早期颞下颌关节不可复性盘前移位患者28例(35侧),用2%利多卡因和维生素B12混合溶液反复灌洗,每天1次,连续3d。治疗后平均随访3.2个月(1～15个月),记录患者疼痛、张口度等相关情况。结果治疗结束后所有患者的疼痛均有较大程度缓解或消失,张口度大幅改善,26例患者张口度恢复正常,但17例仍有关节弹响。总有效率为92.86%。结论关节腔灌洗术能够对部分早期不可复性盘前移位起到积极的治疗作用。

Arthocentesis for acute TMJ disk displacement without reduction

Objective This clinical study is aimed to discuss the treatment and its prognosis of arthocentesis for acute TMJ disk displacement without reduction. Methods From May 2006 to May 2007， 8 cases of acute TMJ disk displacement without reduction，including 1 male and 7 females， with an average age of 26.9 years， were referred to our department. All patients presented a history of TMJ clicking of various durations， and complained of limited month opening and TMJ pain within a week. Arthrocentesis was performed via a preauricular percutaneous method， with a mixture of lidocaine and vitamin B12， once per day for 3 days. The average follow-up period was 3.2 months （range， 1～15 months）. Results Pain of all patients have got relief or disappeared in greater degree after treatment, and mouth opening had great improvement. Mouth opening of 26 patients returned to normal, but 17 patients still had symptoms of clicking in joints. The total effective rates were 92.86%. Conclusion Arthocentesis is quite useful for acute TMJ disk displacement without reduction.
