| 摘 要： | 目的：评价胸中上段食道癌三维适形放疗对脊髓、心、肺的影响。方法：对72例胸中上段食道癌患者采用MLC三维适形设野照射,设4个野适形照射野分割治疗,Dt50 Gy后缩野追量至64 Gy,治疗全程保持GTV体积不变,利用三维治疗计划系统剂量体积直方图进行受量分析,同时与常规3野计划进行比较。结果：72例患者均顺利完成全程放疗,41例发生放射性食道炎（56.9%）,经对症处理均好转或痊愈;23例血象下降（31.9%）;4例发生放射性肺炎（5.6%）;2例心包积液（2.8%）;1例左心室肥厚（1.4%）。结论：三维适形放疗靶区适形度好,剂量更均匀,周围重要器官受量低,适形照射更能保护周围重要器官。
