老年患者全身麻醉腹部手术后并发肺部感染的临床研究

目的探讨老年患者全身麻醉下进行上腹部手术术后并发肺部感染的原因,为其预防提供依据。方法以医院2007年1月-2010年12月进行全身麻醉下上腹部手术的1256例老年患者为研究对象,对其发生肺部感染因素及痰菌培养结果进行统计分析。结果≥70岁患者肺部感染率为10.31%,高于<70岁者的5.63%,手术时间>2 h患者感染率为10.46%,高于≤2 h者的5.40%,术后呼吸道分泌物的清理及时者感染率为6.59%,低于未及时清除者的15.69%,术后早期活动者感染率为5.14%,低于晚期活动者的12.34%,吸烟者的感染率为10.54%,高于不吸烟者的4.90%,差异均有统计学意义(P<0.05);痰菌培养以铜绿假单胞菌、大肠埃希菌及肺炎链球菌等为主,分别占27.45%、22.55%、19.61%。结论老年患者全身麻醉下进行上腹部手术后并发肺部感染的发生与多种因素有关,感染病原菌种类较多,治疗过程应引起重视。

Concurrent pulmonary infections in elderly patients after abdominal surgery under general anesthesia:a clinical study
