卡托普利对充血性心力衰竭患者血浆心钠素的影响

目的 :进一步观察卡托普利治疗充血性心力衰竭 (CHF )的临床疗效及其对血浆心钠素的影响 ,并探讨其可能作用机制。方法 :将充血性心力衰竭患者 40例随机分为卡托普利组 2 0例 ,地高辛组 2 0例 ,观察临床疗效、NYHA心功能分级、心胸比例、血浆心钠素 (ANF)等指标。结果 :卡托普利组临床总有效率为 85 .0 % ,与地高辛组 (90 .0 % )无显著差异 (P>0 .0 5 )。两组 NYHA心功能分级均显著改善 (P<0 .0 0 1)。两组心胸比例均显著改善 (P<0 .0 1和 P<0 .0 0 1)。卡托普利组和地高辛组血浆 A NF水平均显著下降 (P<0 .0 1和 P<0 .0 0 1)。结论 :卡托普利具有治疗 CHF的临床疗效 ,该作用可能与纠正血浆 ANF水平的紊乱有关

Effect of captopril on plasma atrial natriuretic factor in the patients with congestive heart failure

Objective: To research effects of captopril on atrial natriuretic factor in the patients with congestive heart failure(CHF) and to elucidate its possible mechanism. Metheods: 40 cases of CHF were at random divided into 2 groups (captopril group 20 cases and digoxin group 20 cases). Observation of the total clinical effective rate, classification of functional capacity of the heart of the New York Heart Association (NYHA), the proportion of heart to chest and plasma atrial natriuretic factor. Results: It was found that the total clinical effective rate of the captopril group was 85.0%, which showed no significant difference from the digoxin group 90.0%( P >0.05), Classification of functional capacity of NYHA between captopril group and digoxin group after treatment was decreased( P <0.001). The proportion of heart to chest between captopril group and digoxin group after treatment was decreased( P <0.01 and P <0.001, vs before treatment respectively). The level of plasma atrial natriuretic factor in the captopril group was decreased ( P <0.01, vs before treatment), and that of the digoxin group was also decreased( P <0.001, vs before treatment). Conclusion: There is effect on treatment of CHF with captopril. The effect may be due to redress of disorder level of plasma atrial natriuretic factor in the patients with congestive heart failure.
