MSCT在耳颞部骨折中的临床应用研究

摘    要:目的:探讨多层CT(MSCT)高分辨率扫描在颞骨骨折中的成像方法及临床价值。材料和方法:对颞骨骨折30例患者进行MSCT高分辨率扫描,重建数据在Toshiba Aqmilion工作站进行二维、三维图像处理。结果:30例颞骨骨折患者中,发现外耳道及乳突部骨折28耳;鼓室骨折23耳,听小骨损伤9耳;面神经管骨折10耳;内耳骨折3耳;同时骨折累及颈动脉管4耳、海绵窦区及蝶鞍5耳、眶上壁3耳、乙状窦沟及破裂孔各2耳。以上异常改变均行多层面重建(MPR)、容积重建(SVR)、仿真内窥镜成像(CTVE)、X-Proj法及Fly-around法中的一种或几种联合应用进行成像。结论:MSCT高分辨率扫描能清晰的显示外、中、内耳的异常改变,结合工作站强大的图像后处理功能,为临床医生提供准确、直观的三维立体影像,较单层螺旋CT具有明显的优势。

Abstract:Objective: To explore clinical application of multislice CT(MSCT) in temporal bone fracture of the ear. Materials and Methods: In 30 cases(32 ears ) with temporal bone fracture, high-resolution MSCT scan was performed and 2D an d 3D images reconstructions were achieved after volumn data was transferred to w orkstation. Result: In 30 cases, temporal bone fracture, involving the mastoid a nd external meatus was found in 28 ears; attic region in 23 ears, ossicular lesi on in 9 ears; internal meatus in 3 ears and number of facial nerve canal fractur e in 10 ears. In addition, fractures that involved the important structures(caro tid artery canal in 4 ears, cavernos sinus region and sella in 5 ears, sigmoid s inus gap in 2 ears, superior wall of orbit in 3 ears, foramen lacerum in 2 ears) were found in 11 cases. These fractures were all visualized by 2D and 3D images . Conclusion: High-resolution temporal bone MSCT image, combined with 3D post-pr ocessing technology, can clearly show abnormalities of external, middle and inne r ears, and is superior to SSCT.
