B型超声在高血压脑出血微创清除术中及术后的应用

目的 总结并探讨B型超声在高血压脑出血微创置管血肿抽吸引流术中的实时探查及对术后病人床旁动态监测的临床意义。方法 对17例高血压脑出血和6例脑室出血患者进行术中B超引导探查，术后经颅骨缺损处对术区进行动态监测，并且回顾性分析此23例病人的临床资料。结果 19例高血压脑出血病例术中清除血肿70％以上，其中1例出现术后再次出血，经术后床旁B超监测及时发现。6例脑室出血病例中，脑室外引流管均准确位于脑室内，术后复查CT未显示副损伤。23例术前术后CT检查与B超扫查结果基本一致。结论 术中B超引导下行颅内血肿微创清除术，具有定位准确、操作快捷的特点，较传统再出血，迟发性血肿具有重要意义。

Application of B-ultrasound-guided Technology in or After Minimally Invasive Operation of Hypertensive Cerebral Hemorrhage

Objective To discuss the therapeutic effect of B-ultrasound minimally invasive operation in treatment of hypertensive cerebral hemorrhage.Methods23 patients with hypertensive cerebral hemorrhage treated with minimally invasive operation were analyzed based on the methods and prognosis of operation.ResultsRemoving 80% or more hemorrhage in 11 cases,70%～80% hemorrhage in 8 cases,and rehemorrhage 2 cases during operation.The curative effectiveness was assessed according to GOS scores after 3 months:19 recovered well,2 moderately disabled,1 severely disabled and 1 was under vegetative state.ConclusionApplication of B-guided minimally invasive operation in treatment of hypertensive cerebral hemorrhage might be feasible which can help to find transposition of brain tissue and rehemorrhage during operation in time,and to improve prognosis of operation.
