社区不同人群高血压危险因素干预模式探讨

目的探索社区高血压患者管理及高危人群预防的干预模式,控制高血压和预防高血压的发病。方法在社区《居民健康档案》和学校体检的基础上,筛查出高血压患者和高危人员;对高血压患者进行三级管理,对高危人群进行以健康促进为主的综合防制。结果经过两年规范的高血压三级管理及采取健康宣教等干预措施,干预组血压平均控制率达标为42.4%;社区高危人群监测后的血压平均值稳定在正常范围128/79mmHg;青少年对高血压病危害性的认知率有大幅度提高。结论开展高血压三级管理与健康促进和教育相结合的综合干预,是高血压的(治疗)和高危人群的预防应采用的有效措施。实践证明,预防高血压,应从青少年期就开始抓起,这是一个低投入高成效的好方法,值得推广。

Investigation of Intervention Models of Hypertension in Communities

Objective To investigate models of patient management and intervention of high-riskpopulation of hypertension,and further control and prevent hypertension. Methods The patients and high-risk population of hypertension were selected according to the health records of community residents and the results of health examinations in schools.Three-grade management was applied to patients;and integrated treatment and prevention focusing on health promotion were applied to high-risk population.Results Through regular three-grade management and intervention treatments such as health education,the everage rate of bblood pressure control in the intervention group was 42.43%,and the average blood pressure of high-risk population was 128/79mmHg which fell in the normal range.Moreover,the cognitive rate of adolescents for hypertension significantly improved.Conclusions Three-grade management and integrated treatments combining health promotion and health education are effective for hypertension prevention,which is low-input but high-benefit and should be conducted when one is in the adolescent.
