湘潭地区尖锐湿疣人乳头瘤病毒感染的分型研究

摘    要:目的了解湘潭地区尖锐湿疣(Condyloma Acuminatum,CA)患者感染人乳头瘤病毒(hunan papilooma virus,HPV)的基因类型及分布。方法分别用原位杂交(In situ hybridization,ISH)和免疫组化(immunohistiochemistry,IHC)方法对临床和/或病理组织学诊断CA的266例病例进行检测。结果 IHC检测HPV-P阳性166例,HPV-16阳性32例,总阳性率为65.41%(174/266),阳性细胞主要分布在棘细胞上层及颗粒细胞层;ISH检测HPV6/11阳性235例,HPV16/18阳性42例,HPV16/18阳性的42例中有24例合并有HPV6/11阳性。ISH总阳性率为95.11%(253/266),HPV6/11、HPV16/18同时感染57.14%,阳性细胞广泛出现在棘层中上层、颗粒层及角化不全细胞核内。结论本地区CA以HPV6/11感染为主,少数为高危型HPV16/18型;有较高的混合感染率。两种检测方法相比较,原位杂交技术阳性率和阳性细胞数均显著高于免疫组化技术,因此是诊断和研究CA的较好和先进的方法。

Abstract:【Objective】To explore the genotype and distribution of human papilloma viruses among infectious patients with condyloma acuminatum(CA) in Xiangtan region.【Methods】266 specimens diagnosed by clinincal pathol-ogy were examined with ISH and IHC.【Results】Of all specimens,166 cases were positive for HPV-P and 32 positive for HPV-16 by IHC.Overall positive rate is 65.41%(174 /266).Expression of HPV-P and HPV-16 was evident in the upper spinous and granular layer.235 cases were positive for HPV6 /11 and 42 cases were positive for HPV16 /18,24 of which were positive for HPV6 /11 by ISH.The overall positive rate of HPV expression by ISH is 95.11% and the positive rate of double infections(HPV6 /11 + HPV16 /18) analyzed by ISH was 57.14%.In addition to the upper spinal stratum,granular stratum and parakeratotic necleus.【Conclusions】The genotype of HPV6 /11 are the main type as well as little HPV16 /18 causing CA in Xiangtan region.Compared to IHC,ISH had a higher positive rate and more positive cells in continuous section.
