自由呼吸实时导航冠状动脉磁共振血管造影临床应用研究

摘    要:目的研究自由呼吸状态下应用三维导航技术行冠状动脉磁共振血管造影(CMRA)的成像技术,并评价此技术在缺血性心脏病方面的诊断价值。方法运用1.5T超导型MR扫描仪和心脏相控阵线圈,自由呼吸下状态下,应用脂肪抑制、心电向量门控、真性稳态快速进动回波成像(B-TFE)序列多角度双斜面扫描,结合三维自动导航技术(3D—SELF—Navigator)对35例受检者进行CMRA成像。结果全部受检者均能合作完成检查。左冠状动脉主干、右冠状动脉近中段显示满意率为97.14%(34/35),CMRA总显示成功率为91.42%(32/35)。结论自由呼吸实时导航结合B—TFE序列CMRA对冠状动脉主干及主要分支显示满意,可初步用于临床缺血性心脏病患者的筛选检查。

Abstract:Objective To study the effect of free breathing in combination with 3D self-navigator technology on CMRA and to assess the diagnostic value of the technology for ischemic heart disease.Methods CDMA was performed in thirty-five subjects under free breathing and fat suppression,by using a 1.5T MR imager and phasedarray coil of the heart in combination with 3D-self-navigator.Results The CRMA of free-breathing combined with self-navigator was non-traumatic and avoided breath hold,could be replicated and widely adopted.All examinees tolerated the procedure quite well.The rate of satisfactory visualization of LM and the proximal and middle segments of RCA was 97.14%(34/35);the total successful rate of CMRA was 91.42%(32/35).Conclusion The CMRA of free-breathing and self-navigator in combination with B-TFE sequence can visualize the LM and the main branches of the coronary artery,meet the preliminary requirements for screening ischemic heart disease patients.
