拉萨藏族人群尿电解质及氨基酸排泄与血压水平的关系

摘    要:随机抽取拉萨市49—54岁年龄组108名(男性42人,女性66人)藏族人群,测定身高、体重、血压及24小时尿钠、钾、钙、镁和氨基酸等生物学指标,以评价藏族人群独特的饮食习惯与血压水平之关系.结果显示:平均收缩压、舒张压、尿钠和钠/钾比高;尿钾、钙、镁、牛磺酸及1—甲基组氨酸排泄量低.尿镁与收缩压呈负相关;3—甲基组氨酸与舒张压呈负相关.尿镁及3—甲基组氨酸作为独立因素分别进入收缩压及舒张压多元回归方程.收缩压、舒张压与尿钠呈负相关、与体重指数呈正相关,但相关系数均无显著意义.结果提示:饮食中的多种营养因素包括低钾、低镁和低3—甲基组氨酸摄入可能与血压水平和高血压患病率高有一定关系.改善饮食结构可有利于正常血压的调节和高血压的防治.

Abstract:A study, as a part of the international cooperative CARDIAC study, was conducted on Tibetan population in Lhasa to test the specific hypothesis linking the intake of sodium, potassium, calcium, magnesium, protein, and amino acids with blood pressure level. One hundred and six Tibetan descents (42 men, 66 women) aged 49 -54 were selectde rondomly for blood pressure, body weight and height measurements,24-h urine collection, and blood samples obtained according to the design and methods of the CARDIAC study protocol. The samples were analyzed at the CARDIAC study center in Izu-mo, Japan. The mean level of SBP (17. 3 / - 2. 88 KPa), DBF (10. 9 /- 1. 89 KPa), urinary sodium excretion (254. 9 /-101. 3mmmol/day, 14. 9 /-5. 92 g/day), sodium/potassium ratio (7. 8 /- 3. 8) were high. Urinary potassium (38. 9 /- 21. 3 mmol/day), calcium (121. 5 /-61. 4 mg/ day), magnesium (85. 8 / -36. 9 mg/day), taurine (261. 5 /-61. 4 mg/day), and I -mehis (74, 9 /- 42. 3 umol) were low. Urinary Mg was negatively correlated with SBP (P<0. 05) and DBP (0. 1>P>0. 05), 3 - Mehis wsa inversely correlated with DBP (P<0. 05)and both Mg and 3- Mehis were in the multiple regression equation. Urinay sodium was inversely correlated with BPS though the correlative coefficints were not significant. it's likely that multiple risk factors including low k, Mg , and 3 -Mehis intake may contribute to the higher blood level and prevalence rate of hypertension in Lhasa, modifying the components in diet may be hypertension in Lhasa, modifying the components in diet may be benfical to the normal regulation of blood pressure. blood pressure, hypertension.
