限制性门腔静脉侧侧分流术临床研究

限制性门腔静脉侧侧分流术是在门腔静脉侧侧分流术的基础上,通过对吻合口径的限制,在有效地降低门静脉压力的同时,保持一定的门静脉向肝血流量,维持一定的肠系膜静脉压力,以便在达到迅速、有效、持久地控制出血目的的同时,尽可能减轻对肝细胞功能的损害,减少术后肝性脑病的发生。自从60年代我们提出该术式以来,通过临床和实验研究对该术式进行了不断的完善和改良,并于近年又提出了附加限制环的限制性门腔静脉侧侧分流术以及附加限制环的限制性门腔静脉侧侧分流术加肝动脉强化灌注术,通过努力大大降低了术后脑病的发生率。

The Limited Side - to - side Portacaval Shunt

The aims of limited side - to - side portacaval shunt are to improve portal perfusion and maintain mesenteric venous pressure in order to lighten the damage of hepatocellular function and reduce the occurance of postoperative hepatic encephelopathy while decompression of portal system and long - term control of bleeding through limiting the calibre of portacaval shunt, since we proposed this operative procedure in the 60thlast century, we have used it widely in clinical practice and modified it many times, and in recent years we proposed the limited side - to-side portacaval shunt with a limitary ring attached and the limited side - to - side portacaval shunt with a limitary ring attached plus enhancement of perfusion of hepatic artery respectively, through these efforts, the incidence of hepatic encephalopathy after the procedure has been decreased significantly.
