胫骨萎缩性骨不连骨折局部DSA的初步应用

摘    要:目的研究胫骨萎缩性骨不连骨折区局部血液循环的改变情况。方法对12例骨折不愈合患者的骨折局部进行数字减影血管造影(DSA)检查,观察骨折不愈合处及周围软组织的血管形态变化、血管数目变化以及局部血液微循环时间的变化。同时观察12例正常人相应部位的DSA表现,对所得的2组数据进行统计学分析。结果骨不连患者的骨折局部的血管形态无明显变化,12例患者骨折区的血管计数均在正常范围之内,局部血液微循环时间2组比较具有显著的统计学差异(P<0.01)。结论研究表明,局部微循环障碍导致的供氧不足可能是骨不连的一个重要原因。

Abstract:Objective To study the state of blood circulation in the atrophic fracture non-union region of tibia. Methods Twelve patients with tibia atrophic fracture non-union were undergone digital subtraction angiography(DSA) of fracture region. The morphologic changes and the number of vessel and the time of microcirculation in the fracture region were observed. At the same time, 12 cases without fracture accepted DSA in the corresponding region. The data of these two groups were analyzed statistically.Results There was not significant vascular morphologic changes. The number of vessel was in the normal range. The microcirculation time was significantly longer in the fracture non-union group than that in the mormal group(P<0.01). Conclusion This study shows that the regional insufficient oxygen supply caused by microcirculation disturbance may be an important reason for atrophic fracture non-union.
