早期周围型肺癌行胸腔镜手术后炎症反应及免疫功能的变化

目的:分析早期周围型肺癌行胸腔镜手术后患者炎症反应及免疫功能的变化。方法:分析2016年2月至2019年2月收治的400例早期周围型肺癌患者的临床资料,通过随机数表法将患者分为对照组与观察组,每组200例,对照组行开胸手术,观察组行胸腔镜手术。统计两组手术时间、术中出血量、淋巴结清扫数量、住院时间等术后一般情况。检测两组患者血清中炎症因子、免疫球蛋白含量,同时检测T淋巴细胞亚群分布比例。结果:观察组术中出血量、住院时间优于对照组(P<0.05);术后第7天,观察组TNF-α水平低于对照组(P<0.05),IgG、CD3^+、CD4^+、CD4^+/CD8^+水平高于对照组(P<0.05),CD8^+低于对照组(P<0.05)。结论:胸腔镜手术利于早期周围型肺癌患者的术后恢复,并在降低患者体内炎症反应水平的同时改善患者免疫功能。

The changes of inflammatory response and immune function in patients with early peripheral lung cancer undergoing thoracoscopic surgery

Objective:To analyze the changes of inflammatory response and immune function after thoracoscopic surgery in patients with early peripheral lung cancer.Methods:A clinical study was conducted on 400 patients with early peripheral lung cancer from Feb.2016 to Feb.2019.Patients were divided into control group and observation group by random number table method,with 200 cases in each group.The control group underwent thoracotomy,while the observation group underwent thoracoscopic surgery.The operation time,intraoperative blood loss,number of lymph node dissection and hospitalization time of the patients in the two groups were counted.The serum levels of inflammatory cytokines and immunoglobulin in the two groups were detected,and the distribution proportion of T lymphocyte subsets was detected.Results:Compared with the control group,the intraoperative blood loss and hospital stay in the observation group were significantly reduced(P<0.05).TNF-αlevel in the observation group 7 d after operation was significantly lower than that in the control group(P<0.05).The levels of IgG,CD3^+,CD4^+and CD4^+/CD8^+in the observation group 7 d after operation were significantly higher than those in the control group(P<0.05).Compared with the control group,CD8^+in the observation group 7 d after operation was significantly decreased(P<0.05).Conclusions:Thoracoscopic surgery is beneficial to postoperative recovery in patients with early peripheral lung cancer,improves immune function and reduces inflammatory response in patients.
