经脐单孔腹腔镜卵巢囊肿剥除术的临床应用

目的:探讨经脐单孔腹腔镜卵巢囊肿剥除术的临床应用效果。方法:选取2017年11月至2019年4月收治的82例行卵巢囊肿剥除术的患者,随机分为观察组(n=41,采取经脐单孔腹腔镜卵巢囊肿剥除术)与对照组(n=41,采取传统腹腔镜卵巢囊肿剥除术),对比分析两组手术相关指标、术后疼痛评分、并发症发生情况及美容满意度评分。结果:两组术中出血量、术后血红蛋白下降幅度、术后排气时间、住院总费用及住院时间差异无统计学意义(P>0.05),观察组手术时间长于对照组(P<0.05),术后24 h、72 h疼痛评分低于对照组,美容满意度评分高于对照组(P<0.05)。术后两组患者卵泡刺激素、促黄体生成素水平升高,雌二醇含量下降,与术前相比差异有统计学意义,组间差异无统计学意义(P>0.05),观察组并发症发生率低于对照组(P<0.05)。术后2个月,观察组在躯体健康、心理健康、社会功能、物质生活状态方面优于对照组(P<0.05)。结论:经脐单孔腹腔镜卵巢囊肿剥除术是安全、可行的,可达到良好的美容效果,减轻术后疼痛,并发症少,具有临床推广价值。

The clinical application of the transumbilical single-port laparoscopic ovarian cystectomy

Objective:To explore the clinical value of single port laparoscopic ovarian cystectomy.Methods:Eighty-two patients who underwent ovarian cystectomy from Nov.2017 to Apr.2019 were selected,the patients were randomly divided into single port group(n=41,single port laparoscopic ovarian cystectomy)and control group(n=41,traditional laparoscopic ovarian cystectomy),the surgery related indicators,postoperative pain score,complications and cosmetic satisfaction scores were compared between the two groups.Results:In the two groups,there was no significant difference in the amount of bleeding,postoperative decrease of hemoglobin,postoperative exhaust time,total hospitalization expenses and hospitalization time(P>0.05).The operation time of the single port group was significantly longer than that of the control group(P<0.05).Compared with the control group,the pain score at 24 h and 72 h in the single port group was significantly lower and the cosmetic satisfaction score was significantly higher(P<0.05).After treatment,levels of follicle-stimulating hormone and luteinizing hormone in the two groups increased,while the estradiol decreased,compared with levels before operation,there were significant differences;there were no significant differences between the two groups(P>0.05).The incidence of complications in the single port group was lower than that in the control group(P<0.05).After 2 months of treatment,the physical health,mental health,social function and material life of the observation group were significantly better than those of the control group(P<0.05).Conclusions:Single port laparoscopic ovarian cystectomy is safe and feasible,can achieve good cosmetic results,can significantly reduce postoperative pain with fewer complications,and has the value of clinical promotion.
