磁共振灌注成像在脑膜瘤病理分型中的价值

摘    要:目的:探讨磁共振灌注成像(perfusion-weighted imaging,PWI)在脑膜瘤病理分型中的价值.方法:对2003年5月-2006年3月本院36例脑膜瘤患者行术前磁共振灌注成像,计算肿瘤实质平均相对脑血容量(relative cerebral blood volume,rCBV)及最大相对脑血容量值(relative cerebral blood volume,最大rCBV).根据术后病理分为3型(上皮型脑膜瘤,14例;纤维型脑膜瘤,15例;恶性脑膜瘤,7例),分析比较三种类型脑膜瘤rCBV.结果:脑膜瘤肿瘤实质灌注均较正常半卵圆中心脑白质增高,上皮型脑膜瘤的rCBV大于纤维型及恶性脑膜瘤(P<0.05),纤维型脑膜瘤、恶性脑膜瘤的rCBV差异无统计学意义(P>0.05).结论:磁共振灌注成像能够评价脑膜瘤微血管生成情况并对脑膜瘤分型提供依据.

Abstract:Objective:To explore the value of MR perfperfusion-weighted imaging in pathological subtyping of meningiomas.Methods:Thirty-six meningiomas were assigned to three types(meningotheliomatous meningiomas,n=14;fibroblastic meningioma,n=15;malignant meningioma,n=7).MR perfusion-weighted imaging was performed by using GRE-EPI perfusion sequence before resection.Mean relative cerebral blood volume(mean rCBV) and maximum relative cerebral blood volume(maximum rCBV) in parenchyma of meningiomas were calculated.Data were analyzed in combination with pathology.Results:Mean rCBV and maximum rCBV in meningotheliomatous meningiomas were statiscally significant higher than those in other subtypes(P<0.05).There was no significant distinction between fibroblastic meningioma and malignant meningioma in terms of rCBV(P>0.05).Conclusion:MR perfusion-weighted imaging demonstrated valuable evidences for evaluation of pathological types and microvasculature of meningioma.
